What to do if you have an accident?

Below is a list of information one should know & exchange with the other driver, after an accident. We suggest you print this accident information form and keep it in your glove box, along with a pen.

Be sure to get...

•  Driver's name
• Address
• Phone number
• Driver's license number and state of issuance
• Name of insurance company
• Insurance policy number
• Vehicle make, model, and year
• Vehicle identification number
• Registration number and state of issuance
• Vehicle owner's name, address, and number if different from the driver
• If possible, take photos of the entire scene, damaged vehicles and property
• Obtain witness names, addresses and phone numbers
• File an official accident report with the police
• Notify your insurance company immediately
